• None of the levels includes a maximum annual benefit

Also, offer TRADITIONAL PLANs with major carriers.

These plans include:

• 100%coverage for routine care
• 80% & 50% coverage for Basic and Major services (some include a small deductible)
• Annual maximum benefits amounts ranging from $1000/year to $2500/ year
• Waiting periods range from 6-12 months. Some plans don’t have waiting periods and offer graduated benefits instead.
• Options ranging from $28-$95.
• Some plans offer rates based on age
